  • You will not be charged for vaccine, vaccine administration or COVID-19 testing. If you are insured, some charges will be billed to your insurance. Co-pays and deductibles are waived for this service. If this service was received in conjunction with other services, they will be billed to your insurance and may impact your deductible and/or co-pay.

  • We will bill your insurance for the administration fee, but there will be no out-of-pocket cost to you.

    OR
  • No, there will be no out-of-pocket cost for you for the vaccine or testing. However, if you receive an assessment or treatment subsequent to the COVID-19 vaccine or testing, you will still be responsible for any of those charges.

  • We still need to verify your demographic information for reporting purposes and alternate payment source verification. You won't receive a bill for these services.
