
At The Polyclinic, we understand that learning about Medicare options can be an overwhelming experience. The following information has been developed to help you understand your options.
It’s time to plan:
First, find out which health plans your doctor accepts. You can do this by speaking with your doctor at your next appointment, clicking on the Medicare Advantage at The Polyclinic tab, calling our Medicare Advantage Information Line at 206-860-4718, or by emailing us at medicareadvocate@polyclinic.com.
Things to consider:
Some things we encourage you to consider when choosing a Medicare health plan include:
- Can I continue seeing my Polyclinic Primary Care Physician on the health plan I am considering?
- Are my prescriptions covered by this plan?
- Is my pharmacy in this plan’s network?
- Am I able to get the care I want for a cost I can afford?
- Does this plan include the benefits that are most important to me?

Physicians Care Network
Physicians Care Network is the managed care subsidiary of The Polyclinic and oversees and assists in coordinating the care management, claims processing and member services for enrollees of the Medicare plans: AARP MedicareComplete insured through United Healthcare; Humana Gold Plus Medicare Advantage plans and Soundpath Health.
The Physicians Care Network is open Monday – Friday, 08:00 – 4:30 pm. The Care Management team uses Milliman criteria. There are no rewards or incentives for UM decision making. Criteria for determinations are available upon request. If there is an adverse determination, the Medical Director is available for a peer to peer discussion.
