Diabetes Management and Education

The Polyclinic has expert staff and comprehensive services to care for individuals with diabetes.  We emphasize education and self-management in the treatment of diabetes in all aspects of patient care. Medical management is provided by primary care physicians as well as endocrinologists. Individual appointments with certified diabetes educators (nurse and dietitians) are also available in addition to our comprehensive Diabetes Self-Management Education (DSME) program (See Classes for more information).

To assure that our practices are effective, we track the progress of our patients with diabetes by participation in the Chronic Disease Electronic Management System (CDEMS), a system designed to help medical providers track the care of patients with chronic health conditions.  Information is entered, reported and monitored to measure quality improvement efforts.
