We want our patients to see their physician as soon as possible. Most Polyclinic offices are open between the hours of 8:00 a.m. - 6:00 p.m. Monday through Friday, but please check with your physician’s office to confirm.

New Patients can call 206-329-1777 to speak with a New Patient Representative who will help you choose a physician to meet your needs. Our lines are open Monday through Friday, 8:00 a.m. - 6:00 p.m.

Please fax medical records requests to 206-860-4513. Note: requests can take up to 15 days to process. Please indicate the urgency of the request when necessary.
