Now through March 31

Do you think a Medicare Advantage health plan means limited access to medical services and specialists?  Think again.

The Polyclinic’s Medicare Advantage health plans include:

  • Access to specialists in the Swedish network as well as The Polyclinic
  • Plans with Part D prescription coverage
  • Annual physicals, preventive care and worldwide emergency coverage

During Open Enrollment, January 1 – March 31, the Centers for Medicare and Medicaid Services (CMS) allow members to switch one time only from one Medicare Advantage health plan to a “like” Medicare Advantage health plan.  In addition, patients who are utilizing Original Medicare with a Medicare Supplement along with a stand alone Prescription Drug Plan (PDP) may also enroll in a Medicare Advantage health plan.

The Polyclinic has several resources to help patients 65 and older learn more about their Medicare options, including information on the four Medicare Advantage plans accepted by The Polyclinic.  Patients are invited to:

  • Attend a community information meeting sponsored by one of the health plan carriers (meeting times and locations posted at www.polyclinic.com/medicareadvantage)
  • Stop by the Medicare Advantage Information Desk in the lobby of The Polyclinic First Hill, Monday – Friday, 9 am – 3 pm
  • Call Ucentris Insured Solutions, an independent agency assisting Polyclinic patients in comparing plans, at 1-877-217-3279
  • Call The Polyclinic’s Medicare Advantage Information Line at 206-860-4718
