We want our patients to see their physician as soon as possible. Most Polyclinic offices are open between the hours of 8:30 a.m. – 5:00 p.m. Monday through Friday, but please check with your physician’s office to confirm.

New Patients
New Patients can call 206-329-1777 to speak with a new patient representative who will help you choose a physician to meet your needs. Our lines are open Monday through Friday, 7:00 a.m. - 7:00 p.m.

Medical Records
For medcial records inquiries and requests, please contact us at 206-329-1760. A signed release is required for requests. Please see our patient forms for a medical records release form. Requests may be faxed to 206-860-4513.
